NGAL检测的作用：NGAL( neutrophil gelatinaseassociated lipocalin，)中文翻译为“中性粒细胞明胶酶相关脂质运载蛋白”，是人脂质运载蛋白家族中的一个新成员，是近年来新发现的一种早期肾损伤诊断的标志物。在急性肾功能损伤时，血、尿NGAL浓度通常会迅速升高，2 小时即可比临界值升高几十至几百倍，较目前常用的传统指标血清肌酐、尿酶等能够更早地发现肾功能损伤。在慢性肾病患者中，血清中NGAL的水平与肾小球滤过率成负相关，与血肌酐相比，NGAL能更好地反应肾小球滤过率的水平，对于早期发现、早期治疗慢性肾脏病是一个敏感、无创、特异性高的生物学指标。例如IgA 肾病是最常见的一种以原发性肾小球损伤伴小管间质改变为特征的肾小球肾炎, 其小管间质性损伤在疾病发展过程中起了重要作用，尿NGAL能够较目前临床上常用尿NAG更早地反映肾病小管间质损伤。在狼疮肾病患者中尿NGAL是优于抗dsDNA 抗体滴度的狼疮肾病诊断指标，是狼疮肾病活动性的可靠的预测因子。在药物损伤导致的慢性小管间质性肾炎中，尿NGAL优于尿mAlb，α1-MG 等标志物，能更好地反映肾功能损伤程度。在常染色体显性多囊肾病中，检测尿NGAL 浓度能够评估肾脏病变严重程度。在2型糖尿病肾病患者中血清和尿中NGAL水平明显升高，且对疾病的进展非常敏感，有助于早期评价糖尿病肾病患者的肾功能。NGAL还是较好的肾脏疾病的疗效指标，可以反映血管球性肾炎等肾脏疾病的临床疗效。
